Ream And Run Current Concepts Review Recently Published

Dr. Parsons along with Dr. Frederick Matsen and 9 other shoulder surgeons recently published a current concepts consensus in The Journal of Bone and Joint Surgery (Ream and Run Review).This review was based on weekly discussion that this study group undertook from July – September 2020 in order synthesize the group’s collective experience and outcomes into key elements of patient education, surgical technique and postoperative management. 

The Ream and Run is a partial shoulder replacement of the humeral head only combined with reaming or reshaping of the socket without socket replacement. This operation was pioneered by Dr. Matsen recognizing the need for both a solution shoulder arthritis in the physically demanding patient and the higher potential for socket implant failure in these patients. Reshaping the smoothing the socket allows a near conforming fit with the replaced humeral head and restores stability and load transfer to the shoulder damaged by arthritic wear. By retaining the native socket and rebalancing the soft tissues around the arthritic joint, surgeons can permit patients a restriction-free lifestyle with the potential for long-term durability.

The Ream and Run requires special techniques and instruments that not all shoulder surgeons employ. Good results stem from a combination of understanding the biomechanics of this operation relative to each shoulder and from shepherding patients through the recovery. Patients interested in this option for shoulder replacement must be highly motivated to do the rehabilitation necessary to achieve an optimal outcome. Dr. Parsons was fortunate to study under Dr. Matsen during his shoulder fellowship and continue participating in this study group.
